Conveniently located at 3610 Old Hwy 53 in the unique Geodesic Dome building, Jack Elizares OD is just up the road from the RiteAid store off Olympic Drive in Clearlake, CA.

Our Office is open Monday through Thursday from 9 a.m. to 5 p.m.
We are closed for the lunch hour between 12:00 and 1:15 p.m.
